Credit: WINK News. Florida Department of Health tweeted Tuesday night the “COVID-19 Determination of Extreme Vulnerability” form for those under 65 seeking to get approval for vaccination. It must be signed by a doctor after a patient has been determined to meet the criteria defined by the health department as someone who is “extremely vulnerable” to COVID-19. Gov. Ron DeSantis’ signed an executive order that allows Floridians under 65 who are considered “extremely vulnerable” to COVID-19 to get vaccinated at store pharmacies.
