WASHINGTON (AP) - A group of technical experts says BP and its contractors missed and ignored warning signs before the massive oil well blowout in the Gulf of Mexico.
In a report released last night, the panel convened by the National Academy of Engineering says the companies failed to learn from "near misses." And it says neither BP nor its contractors caught or corrected flawed decisions that contributed to the blowout -- and that federal regulators missed them as well.
